You can sign up via one of the links to SignupGenius to schedule an appointment for your chosen clinic.
If you need to cancel your appointment for some reason, please call the Student Life Office at x4332 to notify us.
Appointments are only 5 minutes in length, and there are a limited number of time slots.
There will be no walk-ins for flu shots this year.
You must complete your daily symptom tracker prior to coming for your appointment (and you cannot have any symptoms).
You must wear a mask to your flu shot appointment.
Please arrive 5 minutes prior to your appointment to complete the necessary paperwork.
Your temperature will be taken prior to receiving the flu shot.
There are 3 options of payment for the flu shot:
Bill your insurance. **You MUST bring your insurance card, so Walgreens can process the billing.
Bill your CUW account
Cash $30
